Through our affiliation with Jewish Home, we’re able to connect patients with an in-home outpatient therapy option that addresses exactly that.

Jewish Home’s Rehabilitation Wellness Home Physical Therapy program sends licensed physical and occupational therapists directly into patients’ homes throughout Monroe County. For many of our patients, this removes the single biggest obstacle to staying consistent with a therapy plan.

Why we refer patients to this program

We know our patients. We know that for some, a trip to a clinic, especially in winter, or after a procedure, or while managing a condition like Parkinson’s, is genuinely difficult. When therapy happens at home, patients tend to stay engaged longer and get more out of each session because they’re comfortable, and because the therapist is working within the actual environment where independence matters most.

Therapists through this program specialize in older adults and work with patients on strengthening, balance and gait training, fall prevention, and daily activities like getting dressed, moving between rooms, or managing stairs. They can also flag simple home modifications like grab bars, ramps, and better lighting that make a measurable difference in safety and confidence.

What you should know before getting started

An evaluation happens within 48 hours of the first call. From there, a care plan is built around the individual patient’s goals and living situation, not a generic protocol.

The cost structure mirrors standard outpatient therapy at a clinic, so private pay, insurance, and co-pays work the same way. Patients can also move to in-person clinic sessions at any point if that becomes preferable as their health improves.
