Proactive care coordination that helps older adults in their homes
Jewish Home has launched Living Well Connections, a proactive care coordination membership program designed to help older adults remain independent, informed, and prepared for whatever the future may bring. The program, the first of its kind in New York State, focuses on helping members create a personalized roadmap for aging before urgent needs develop.
It is the latest addition to Jewish Home’s growing portfolio of aging-in-place services known as “Jewish Home At Home”. These services are designed to help older adults continue living safely and comfortably in their own homes while accessing the support they need. At-Home services include home care, physician house calls, tech support, and home physical and occupational therapy.
“Our goal is to help you remain independent while knowing you have an experienced team in your corner before a crisis arises, ” said Alana Russell, LCSW, Program Director and Care Coordinator. “Your care coordinator serves as an educator, guide, advocate, and partner, helping you navigate decisions and understand options. Together, we plan ahead and our support can adjust as your needs change.”
Members work closely with a personal care coordinator who helps them develop and maintain an individualized care plan based on their health, lifestyle, goals and future needs. The plan evolves over time, adapting as circumstances change while keeping the member at the center of every decision.
Every membership includes:
- a dedicated local care coordinator
- personalized wellness and care planning
- non-clinical assessments of daily living and cognitive health
- preventive support focused on maintaining independence
- advocacy and guidance every step of the way
By combining expert guidance, personalized planning, advocacy and ongoing support, Living Well Connections gives older adults and their families a practical way to prepare for the future while continuing to enjoy life today.
